Published by Skylight Paths in 2008, my book is available widely through Skylight, Amazon, Barnes and Noble, Borders, and other booksellers.
You can read a lot of it at Google Reader, or find it in libraries through the same site. There are a couple of copies in the Chicago Public Library, which was fun for me to discover!

Hi Nanette, I receive the JOPA e-mails and thus learned about some of the work that you’ve been doing. I would love to come to the Church Planter’s Academy in May, but I think the pressure of school and money might hold me back. I’m a student at Luther Seminary studying to be a Diaconal Minister and in my field work this year in rural Northwoods MN I have discovered what a problem hospitality is and am working to find ways to educate the church and implement strategies to be more hospitable. More specifically I am focused on children, youth, and family ministry for my lens. I look forward to reading your book and hope that it helps with my studies and field work. Thank you for your work!
